I understand that La Belle Vie Fitness & Dance/La Belle Vie Dance School & Co. (LBV), its owner, director, and instructors are not responsible for accidents, injuries including death, or damage or loss that may occur to me or my child on LBV premise or during any rehearsals or performances associated with LBV. I understand that a class trial does not count as registration for a full year of dance and that a formal registration (plus the required fee) will be required should I decide to continue lessons with LBV.
Our teachers at LBV are highly trained and are required to be certified in First Aid/CPR/AED in case of emergencies. I give permission to LBV, its owners, operators, and staff to seek medical attention in the event contact cannot be made with parent/legal guardian. I will ensure that I have notified LBV and associates of any health conditions that could affect the training of my child.
I also understand that LBV retains the right to cancel or reschedule classes with less than three students enrolled. LBV and associates will give sufficient notice if any classes are cancelled.
I understand that videos and/or photos may be taken of students during LBV classes or programs. By signing this, I give permission for my image or my child’s image to be used at LBV in its promotional materials, videos, or on its website in a limited capacity. By signing, I affirm that I have read and understand all the policies of La Belle Vie Fitness & Dance/La Belle Vie Dance School & Co. and agree to comply with them. I also affirm that I am aware that there is a security camera in the dance room recording classes in case of incident and that this footage is not used in any way as promotional material but for safety only.
